This past weekend, the GRACE Transitional Housing ladies embarked on the De-Clutter challenge! Led by organizer coach Tammy O’Neil, each family was tasked with spending 15 minutes a day gathering all of the items in their homes they no longer needed. These items were then packed up and donated straight to GRACE resale stores! But why clutter? And why start spring cleaning so early? We have been awarded GOLD Plus Recognition for the seventh consecutive year for the Target BP Program through the American Heart Association! In summary, this means that 87% of our clients with a hypertension diagnosis have been able to keep their blood pressure in check per the American Heart Association standards for blood pressure control. The Clinic provides patients with individual physician management for their chronic diseases, medication management, and patient education. GRACE teaches them how to self-monitor their blood pressure daily and provides the tools to do it, i.e., Validated Blood Pressure Machines, Blood Pressure Logs for their self-management, and 1-1 patient education. These patient blood pressure logs are brought to each physician and nurse visit to help providers in their patient assessment and medical management. We educate our clients regarding the importance of blood pressure control
